Pastor/Church Leader Questionnaire - It is the responsibility of the family to complete the family information of this form and send it to your local church pastor or church leader. The church may fax or mail the completed forms to the admissions office, and the family will be notified upon receipt.
After The Forms Are Received
Once all documents are received, the application packet will be sent to the admissions committee for review. Depending on their recommendation, testing (if necessary) and the parent interview will be scheduled at that time.
- TESTING
- Students applying for the Pre-K and/or Kindergarten program will be screened with a standardized evaluation instrument. This instrument assesses language and literacy development, number concepts, motor skills, and social development. Students must score at or above the developmental level. Parents will be notified of the screening results and whether the student is eligible for the next step in the admissions process.
- Students applying for grades 1-12 must submit the results of a nationally-normed standardized test, taken within the last 12 months. Eligible students must score above average or at grade level on the nationally-normed standardized test or at the “exceeds” level of the CRCT. If current testing records are not available, a testing date will be scheduled.
- There is a $50 testing fee per student that can be paid at the time of the test.
- INTERVIEW
If the application has been deemed eligible, an interview will be scheduled. Both parents and all 6th-12th grade students are required for the interview. Students entering PreK – 5th grades are not expected to attend the interview.
- ACCEPTANCE
Acceptance of the applicant is based on review of the application, test scores, transcript grades, the pastor/church leader questionnaire, and interview results if applicable. Loganville Christian Academy does not discriminate against sex, race, age, or national origin. Fall acceptance/denial letters will be issued the last week of February. Late applications may be submitted as enrollment allows.
- REGISTRATION
Registration forms are included in the acceptance letter. A student is not officially enrolled at Loganville Christian Academy until the registration forms, financial contract, and registration fee are received.
- OTHER INFORMATION REQUIRED FOR ADMISSION
- Certificate of Immunization – Form 3231 (revised 2007) – According to current state law, we CANNOT permit a child to begin classes until we have received this form. It may be obtained from your family physician or local health department.
- Eye, Ear and Dental Examination Form – Form 3300 – This may be obtained by your family physician or local health department. State law requires that this form is on file for each student.
- Copy of the birth certificate
- APPLICATION DEADLINE
Students desiring an immediate transfer from area schools during the school year will not be accepted for that current school year after February 1st. This policy does not apply to new students relocating to the area.
